Download the application from the above link, fill it out and turn it in to the front office at UHS with the requested documents.
Applications for the fall semester are accepted up to the 3rd week of the semester. Applications received after that date will be considered for the following semester.
Applications must have recent transcript, attendance report, and discipline logs from your current school.
Applications will be reviewed by administration and the counseling office. Students/Parents will be notified of acceptance or rejection.
